Our bodies wage a continuous war against enemies too small to be seen with the naked eye. If we’re healthy, our immune system will stop the vast majority of these microbial agents.

But some of them, viruses in particular, are so small and with certain characteristics that they can slip past our immune systems. Prevention — removing the opportunity for these viruses to gain entry into our bodies in the first place — is a key component in controlling infection.

Healthcare facilities, including dental offices, are primary battlegrounds in this war. In recent years, the stakes have increased as viral infections that cause the liver disease hepatitis (B and C) and HIV that causes the auto-immune disorder AIDS are on the rise. Although different in effect, these viruses spread in much the same way — when the blood of an infected person comes in contact with the bloodstream of another person.

The risk for this exposure is higher in situations when there’s a break in the skin. Blood transfusion, surgery centers and similar facilities with invasive procedures require high standards of protection to prevent viral transmission between people.  This includes dental clinics — even a routine hygienic cleaning can become a conduit for viral infection.

As a result, the more than 170,000 dental providers across the country have adopted strict infection control standards that conform to the National Center for Disease Control (CDC) guidelines, as well as state and local regulations. These standards detail such issues as wearing protective equipment and clothing (like disposable gloves, gowns or facemasks), cleaning and sterilizing instruments, or disposing of bio-hazardous waste.

High infection control standards are also promoted by the professional boards and organizations of dental providers, like the American Dental Association, and are a requirement for continued membership. As a result, infection occurrences from dental visits or procedures are extremely rare.

With a 95-plus percent survival rate after ten years, dental implants are one of the most durable replacement restorations available. Implants can potentially last much longer than less expensive options, which could make them a less costly choice in the long run.

But although a rare occurrence, implants can and do fail—often in the first few months. And tobacco smokers in particular make up a sizeable portion of these failures.

The reasons stem from smoking’s effect on oral health. Inhaled smoke can actually burn the outer skin layers in the mouth and eventually damage the salivary glands, which can decrease saliva production. Among its functions, saliva provides enzymes to fight disease; it also protects tooth enamel from damaging acid attacks. A chronic “dry mouth,” on the other hand, increases the risk of disease.

The chemical nicotine in tobacco also causes problems because it constricts blood vessels in the mouth and skin. The resulting reduced blood flow inhibits the delivery of antibodies to diseased or wounded areas, and so dramatically slows the healing process. As a result, smokers can take longer than non-smokers to recover from diseases like tooth decay or periodontal (gum) disease, or heal after surgery.

Both the higher disease risk and slower healing can impact an implant’s ultimate success. Implant durability depends on the gradual integration between bone and the implant’s titanium metal post that naturally occurs after placement. But this crucial process can be stymied if an infection resistant to healing arises—a primary reason why smokers experience twice the number of implant failures as non-smokers.

So, what should you do if you’re a smoker and wish to consider implants?

First, for both your general and oral health, try to quit smoking before you undergo implant surgery. At the very least, stop smoking a week before implant surgery and for two weeks after to lower your infection risk. And you can further reduce your chances for failure by practicing diligent daily brushing and flossing and seeing your dentist regularly for cleanings and checkups.

It’s possible to have a successful experience with implants even if you do smoke. But kicking the habit will definitely improve your odds.

The “magic” behind a dental implant’s durability is the special affinity its imbedded titanium post has with bone. Over time bone grows and attaches to the titanium surface to produce a strong and secure hold.

But there’s one important prerequisite for ultimate implant success—there must be an adequate amount of bone available initially to properly position the post during implantation. Otherwise, the implant may not have enough support to position it properly or cover the implant surface completely with bone.

Inadequate bone can be a problem for patients who lost teeth some time before and now desire to an implant restoration. This happens because when teeth are missing, so are the forces they generate during chewing. These forces stimulate new bone growth around the tooth root to replace older, dissolved bone at a healthy rate. If that replacement rate is too slow, the volume and density of bone may gradually diminish.

There is a way, though, to build up the bone for future implantation. Known as bone preservation procedure or a ridge augmentation, it’s a surgical procedure in which the dentist adds bone grafting material to the extraction socket or the bony ridge. The graft serves as a scaffold for new bone cells to grow and multiply. If successful, there will be enough new bone volume after several months of healing to support proper implant placement.

Bone grafting can add more time to the implant process. It may also mean you will not be able to undergo immediate crown placement after implantation (a “tooth in one day” procedure). Instead we would probably suture gum tissue over the implant to protect it and allow for full integration with the bone over a few more months. In the meantime, though, we could fit you with a temporary restoration like a removable partial denture (RPD) or a bonded bridge to improve the appearance of the space while the bone continues to heal.

After several months, your implant will have a better chance of a secure hold and we can then attach a life-like crown. Even if you’ve suffered bone loss, you’ll then have the benefit of not only a durable implant but also a new smile.
